1. REVIEW

  • Training

  • Marketing

  • Management

  • Informative Webinar

  • Video Tour of Clinic

  • "Introduction" Video by Carl Mudd, President / CEO, on the Franchise Information Home Page.



    • 2. REQUEST INFORMATION

    Fill out and submit the Franchise Information Request over the web.



    3. RECIEVE INFORMATION PACKET

    Once we receive your request form, an information packet will automatically be sent to you, including:

  • A Formal Application

  • A Questionnaire

  • A DVD

  • Marketing Samples



    • 4. ONE-ON-ONE PHONE CALL

    Within 24 hours of your request, a member of our Franchise Development or Franchise Support Team will call you to answer any additional questions. You can also:

  • Schedule your Webinar

  • View Webinar

  • Confirm that your desired franchise territory has not already been awarded.

  • Request a copy of the Uniform Offering Circular (UFOC), which includes:

  • The Franchise Agreement

  • Financial Information

  • FTC Mandated Information

  • Request a private Conference Call with CEO/President, Carl Mudd

    5. ATTEND WEBINAR. ATTEND DISCOVERY DAY

    You can attend a Webinar and Discovery Day or just the Discovery Day (though we highly recommend attending both).

    Discovery Day includes:

  • Visiting the Corporate Headquarters

  • Meeting the Executive and Support Team

  • Touring the highly successful Phoenix metro clinics - meeting their owners, seeing their physical plans, seeing them "in action".

    • It is a "mutual" Discovery Day designed to make sure it is a great fit for both of us.



    6. GET PREPARED

    Prior to Discovery Day, you will need to:

  • Submit your detailed application along with a $100 check for your background check.

  • Answer a brief questionnaire

  • Return the signed "Acknowledgement Page" from the UFOC

  • *Review your "Executable" document, including:

  • Your Franchise Agreement and Exhibits

  • Your Territory Description

    • * If you are going to have counsel or a trusted advisor read the document for you, this should be done prior to Discovery Day.
